Forgot Username

This is an optional feature available in portal v4.4.2 and later. It is possible that you do not have this feature in your portal.

On the portal log on page use the Forgot User Name link to have your username sent to you in an email. If you do not see the link, the feaure is not available. For this feature to work, you must have a valid email address associated with your portal account. For additional assistance, click here to contact us.  

  1. On the portal log on page, select Forgot User Name.
  2. The Forgot Username dialog box opens.
  3. Enter your First Name.
  4. Enter your Last Name.
  5. If "I am a" and two option buttons are displayed, select the Provider or other Practice/Hospital staff member option button.
  6. Enter the email address linked to your portal account so you can receive the email with your username.
  7. Click or tap the box to indicate you are not a robot. In some cases you must complete more tests before proceeding. 
  8. Select Submit.
  9. Message box opens confirming submission of your request. Note any instructions.
  10. Access your email account and check your inbox for a new message containing your portal username.

If you don't see the new message, check your spam or junk email folder. If several minutes have passed and you still can not locate the email, follow the instructions in the message box that confirmed your submission (step 9), or click here to contact us.
